The Forever Picture is a great way to preserve the memories of someone who is in the early stages of Alzheimer's disease or Dementia, and are still in a position to remember many details of their life. It provides a way for family members to remember that individual's personality and lifestyle.

IF YOU WANT TO HELP SOMEONE WHO HAS DEMENTIA: The best thing you can do is to help them create their own Forever Picture. To do this, set up a time for you to visit with them. During the visit, interview them about their personality and lifestyle using the narrative questionnaire on this site. While you are there, you can also take their picture!
IF YOU ARE THE PERSON WHO HAS DEMENTIA: The best thing you can do for yourself is to create your own Forever Picture, then send it to members of your family as a gift, perhaps at Christmas, or on their birthday. This will become a cherished family heirloom.
